Username: Your username will be your last name followed by the last 4 digits of your Social Security Number. For example, Joe Wages, SSN 123-45-6789 will have a username of WAGES6789.

Default Password: Your initial temporary password will be be the 4 digit year of your birth followed by the first 2 letters of your first name. For example, if Joe Wages was born on January 1, 1980, his password is 1980JO.
